Change of Information

• Contact Information Changes/Additions - use this form to alert us to phone number changes, email address changes, or non-custodial parent address changes. When a student and their custodial parent moves, they need to complete an FPS Change of Address online Form (see below).

MOVING:

• Student is moving within the town of Franklin
  • You are required to complete the online Franklin Public Schools change of address form and upload the documents we require. You can complete this form once for all of your children that attend Franklin Public Schools
